Job Title: Chronic Disease Nurse / Practice Nurse

Reports to: Nursing Services Lead

Overall Aim: To provide assessment, treatment, screening, health education services and advice, working in accordance with the NMC Code of Conduct.

Nursing Responsibilities:  Provide nursing treatments (following specific training and within agreed protocols and guidelines) to patients independently or in participation with nurse colleagues and general practitioners with reference to agreed protocols and guidelines:

  • Patient Assessment
  • Diabetes Management
  • Respiratory disease management
  • Cardiovascular disease monitoring and management
  • General health promotion including patient self-education

Following suitable training and Practice approval further duties may include:

  • Becoming a Practice expert and help update the Practice policies and protocols in areas of patient care.
  • Blood pressure checks
  • Injections & immunisations (child & travel vaccination), Child immunisation where special responsibility has been given and trained to do so.
  • Undertake the collection of pathological specimens including intravenous blood samples, swabs etc.
  • To act as a “chaperone” when requested by a clinician.

Managerial & Administrative Responsibilities:

Monitor fridge temperatures

Ensure accurate and legible notes of all consultations and treatments are recorded in the patients computerised record

Attend and participate in nurse and practice meetings as required

Maintain good liaison with other surgeries and agencies including secondary care.

Ensure that the Nurse Practitioner/Nurse Manager is informed of any matters arising or problems / potential problems.

To take responsibility for clinical equipment and security

To assist in the movement of equipment and materials as required.

Assist in the production and maintenance of statistical information required for ad hoc reports

Maintain continued education by attendances at courses and study days as deemed useful or necessary for professional development ensuring PREP requirements are met. Develop and maintain a Personal Learning Plan

Arrange ordering of vaccines and general drugs, pathological laboratory stores, patient literature, etc. to maintain sufficient stock, reporting to the Nurse Practitioner/Nurse Manager as appropriate.

To assist in the education of colleagues (HCAs) and medical students where required

 

Key Results: Good communication and relationships must be established and maintained with patients, colleagues and employers & high level practice at all times

Please also be aware of and follow the Surgery’s policies and procedures, with particular attention to patient confidentiality, health and safety, equality and diversity and customer service excellence. To further your development and knowledge you will be expected to attend training as necessary.

All employees in the Surgery are expected to display a high standard of teamwork. This includes co-operating in undertaking work of absent colleagues or assisting others where the workload is particularly high. Accordingly there may be a requirement, from time to time, to undertake other duties.

The Surgery reserves the right to amend this role profile as necessary, after consultation with the postholder, to reflect changes in or to the job.

Person Specification: You would be friendly and approachable, with good interpersonal skills and effective communication.

You are competent and committed to high quality care with an awareness of your own limitations and when to seek advice.

You will be flexible and happy to work as part of our nursing team across two sites as required.

You will be confident to start working effectively from the offset, and willing to undertake necessary training when asked to do so.

You will manage your time well.  Basic computer literacy is essential.  Necessary training and support in clinical skills and clinical documentation systems will be provided.

Experience in Diabetes & Chronic respiratory disease management is essential.

Please note: previous community / general practice experience is beneficial but not essential for this role.
