Practice Nurse, Full Time 

Overview of Role – Main Duties 

At Victor Street you can do your best work. 

As a Practice Nurse you will work as a key part of the surgery multi-disciplinary team.  You will use your skills to deliver important care to our patients, such as immunisation and cervical cancer screening programmes, routine health checks and healthcare for our patients living with learning disabilities as well as day to day procedures such as performing ECGs, blood taking and giving injections.  

You will ensure care is seamless for our patients, and that everyone involved is working together. You will provide the capacity and expertise to support patients who are living with long term medical conditions such as diabetes, COPD, significant mental illness, and learning disabilities, and will support our patients to engage with their health needs to make positive change.  

This role is an essential part of our evolving dynamic and forward-thinking surgery.  You will support people to help them make good choices about their care, using tools to understand people’s level of knowledge, confidence in skills in managing their own health and wellbeing, and work as part of our team to continue addressing health inequalities.   

Alongside our other clinicians such as GPs, ANPs, pharmacists, social prescribers, mental health workers and physios, you will be joining a team who are committed to continued personal and shared education.

Overview of Victor Street 

We serve a community of 12,200 people in the West of Southampton in an urban area, and pride ourselves in delivering patient-first care alongside ambitious and innovative clinical and strategic leadership.  We drive changes from the front: we have a Clinical Director for our PCN, a GP trainer, an LMC rep, and a non-clinical Partner shaping strategy and development. 

Our clinical team consists of 6 GPs, 2 Advanced Nurse Practitioners?who deliver a mix of same day and?long-term?condition care, and do our acute home visits,?and an excellent Practice Nursing Team who take the lead on managing long term conditions.
