We are looking to recruit a motivated and committed Salaried GP to join our practice team. The vacancy is for 6 – 8 sessions per week. • PMS Practice with list size of 7750 • CQC overall rating ‘Good’ • Established training practice • High QOF achievers • SystmOne Clinical System • Single site with purpose built modern premises and free parking We have 3 partners, 2 salaried GPs and full complement of Nursing and Administration staff. We have an excellent support team, including Clinical Pharmacists, Health and Wellbeing Coach and Care Coordinator. We currently train GP Registrars and Foundation 2 Doctors. We have good links with our local neighbourhood nursing team which include District Nurses and Community Matron. We are members of Barnsley Healthcare Federation and Barnsley Primary Care Network. Job Description Provision of general medical services to patients registered with the practices, including surgery and telephone consultations and queries, visiting patients at home, taking part in the on-call rota, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion and as clinically appropriate. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other healthcare workers within the practices. Assessing the healthcare needs of patients with undifferentiated and undiagnosed problems. Screening of patients for disease risk factors and early signs of illness, identify appropriate follow-up and advice in consultation with patients and in line with current practice disease management protocols, developing care plans for health. Providing counselling and health education Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate. Recording clear and contemporaneous consultation notes within the e-record including history taking, examination, investigation, diagnosis, treatment and treatment plans, and referral where appropriate. Collecting data for audit purposes linked to QOF Data Collection Prescribe in accordance with the practices prescribing formulary (or generically) whenever this is clinically appropriate, participation in Primary Care Prescribing Advisor reviews In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care Eg on call duties within normal hours. Other Responsibilities within the organisations awareness of and compliance with all relevant practice policies/guidelines, e.g. prescribing, information governance, clinical governance, health and safety. A commitment to life-long learning and audit to ensure evidence-based best practice. Contributing to evaluation/audit and clinical standard-setting within the organisation. Maintenance of patients clinical e-records in line with NHS legislation and practice protocols, ensuring the system clinical coding is used appropriately, that health summaries are effectively maintained; recording consents to and enablement of appropriate e-record-sharing
