We are thrilled to announce an opportunity for a skilled Clinical Pharmacist to join our team! This role offers the chance to collaborate with healthcare professionals, optimise medication therapy, and improve patient outcomes.
We are seeking a dedicated individual with strong clinical knowledge, excellent communication skills, and a passion for patient-centred care. As part of our multi-disciplinary team, you will play a key role in delivering a high-quality medicines optimisation programme. Your daily interactions will include patients, community pharmacists, care homes, and ICB/PCN pharmacy teams.
Key Responsibilities:
Work as an independent prescriber, providing specialist advice to patients and colleagues
Conduct comprehensive medication reviews, proactively managing complex polypharmacy, particularly in elderly patients and those with multiple conditions
Support the practice in delivering effective and safe prescribing practices
Requirements:
- GPhC Registered Pharmacist
- GPhC Independent Prescriber Qualification
- Experience working in primary care
- Strong IT skills, including working knowledge of EMIS Web
- Excellent customer service and interpersonal skills
