Job summary BD4+ PCN are a PCN in South Bradford supporting people residing in BD4 & BD12. The PCN is made up of 4 practices, Bowling Highfield Medical Practice, Low Moor Medical Practice, Rooley Lane Medical Practice and Tong Medical Practice. The PCN Manager will support the four practices and work independently on all aspects of the PCN DES. The PCN board structure is a GP Partner representation (1 is Clinical Director) and a Business/Practice Manager representative from each practice. The role will require the post holder to work both strategically and operationally with the board to ensure that the PCN is effectively managed. This role will suit a self-starter who can work autonomously and build strong relationships with practices and wider partners.
The post holder will act as the main point of contact, coordinate the successful achievement of all PCN contractual requirements and support collaborative working to achieve improved outcomes for the member practices and local population. They will get involved with wider system work such as Community Partnerships to reduce health inequalities. This is a varied and exciting role to support collaborative working and ensuring that the PCN DES has a positive impact.

Main duties of the job
The PCN is well established with a large ARRS workforce to support Primary Care and you will support on all aspects of the DES. There are subcontracted staff and those employed by practices, and they work on various areas of the DES and supporting Primary Care.

About us
The role will be employed by Tong Medical Practice where the successful candidate will be based. They will also work closely with the other 3 practices which are Low Moor Medical Practice, Rooley Lane Medical Practice and Bowling Highfield Medical Practice and spend time in the other practices. There are roughly 45,000 patients belonging to the PCN and due to its deprivation the adjusted list size is over 49,0000.
The role requires a degree of self-management and can be worked flexibly including an element of working from home to be agreed by the Clinical Director.

Job Responsibilities
Contracts
- Keeping up to date with any updates on NHSE, GP Contract or local contracting, interpreting, and supporting the practices to disseminate information and operationalise the requirements (ie PCN DES, IIF etc).
- Developing strategic and operational plans with the Clinical Director and board members to ensure the DES is met.
- Reporting and monitoring how the PCN is performing against contract requirements including writing and running clinical reports and other reporting functions within SystmOne.
Finances
- Financial management including annual forecasting for ARRS staff and other areas of the PCN.
- Providing quarterly financial updates to the PCN board and liaising with the accountants.
- Operational management of the finances.
- Working closely with practices on their ARRS budgets and ensuring budgets are in line with ARRS guidelines.
- Using PCSE to reconcile finances and ensure any discrepancies are identified.
- Liaising with suppliers and ensuring invoices are paid in a timely manner.
- Submit all ARRS claims and associated workforce document to ICB/NHSE.
Contract Management:
- Manage contract negotiations with partners and ensure best value for money for the PCN.
- Ensure SLAs are in place and other contract requirements such as DSP toolkits, data sharing agreements, etc.
- Contract management ensuring the services are delivered to a high standard and any associated quality improvement required is actioned.
Staff Support
- Support integration of ARRS staff across the PCN.
- Ensure key regulatory policies and procedures are maintained ensuring CQC compliance.
- Tracking of relevant information relating to ARRS staff not employed directly by the PCN such as professional registration, mandatory training, etc.
- Maintain CQRS return for PCN workforce and any other associated documentation.
- Maintain and update rotas (ensuring they are mapped correctly), collate central calendar to identify annual leave, track sickness and leave for training of ARRS staff.
- Induction of new staff and adding to relevant systems (systm one), IT access, Honoury contracts, etc.
- Support new staff in access to systems and facilitating contact with key contacts within each practice and ensuring staff get the clinical support required to work effectively.
- Identify and develop appropriate policies and protocols to be shared across the PCN
Project Development:
- Support and oversee the CMHT VCS Core transformation team as part of the PCN offer.
- Represent the PCN and contribute to the Community Partnership and any other system developments such as Neighbourhood teams where appropriate.
- Responsible for the delivery of service improvement initiatives Support the joint commissioned services such as Covid 19 Vaccination programme.
- Support and oversee any other system work related to the PCN as it emerges.
- Ensure monitoring and evaluation of services within the PCN and support with projects. To develop strong relationship with key members of PCN and practice staff and wider primary care, CCG and external provider staff to ensure PCN effectiveness.
PCN Project management
- Understand and communicate objectives and requirements to practices

Governance
- Ensure the PCN has the appropriate governance in place and this is reviewed periodically or where changes arise. This includes Schedules, data sharing agreements, GDPR, Privacy policies, confidentiality, etc.
- Support development of any oversight of PCN agreements and cost sharing agreements.
General Support
- Support the Clinical Director to set agendas and take minutes of PCN and any other appropriate meetings and ensure they are circulated in a timely manner.
- Provide support to the PCN Leadership Team as required
- Review and maintain policies and protocols
- Keep the PCN informed of any matters arising as necessary, or as requested by other members of the PCN
- Create and run reports relating to IIF/DES or other indicators to monitor progress and achievement of indicators
