We are looking for a dedicated and knowledgeable Clinical Pharmacist to join our forward-thinking and supportive team at Sheringham Medical Practice. Nestled on the picturesque North Norfolk coast and surrounded by beautiful countryside, our practice serves a patient list of 9,400 and provides a full dispensing service to 1,400 patients. Our clinical team includes four partners and a diverse multi-disciplinary group, featuring a first contact physiotherapist, mental health liaison worker, social prescriber, and recovery worker. Additionally, we have a robust nursing and healthcare team with expertise in respiratory, diabetic, and women’s health.

Our Clinical Pharmacist will play a vital role in managing medicines-related queries, carrying out structured patient reviews, and contributing to our Medicines Optimisation agenda. With a well-trained administrative team skilled in managing clinical correspondence, data entry, signposting, social prescribing, and medicines management, our practice is focused on delivering high-quality care and support for both staff and patients.

Role Overview:

The Clinical Pharmacist will act as the primary contact for medicines-related inquiries, conducting in-depth medication reviews, particularly for patients with complex needs. They will perform structured reviews in care homes, assess and treat patients using advanced medication knowledge, and guide patients to appropriate healthcare services when needed. The pharmacist will work closely with GPs on medication updates and solutions.

Prescription Management includes collaboration with dispensary and practice teams to support medicine optimisation goals, promote cost-effective prescribing, manage acute and repeat prescriptions, and ensure adherence to prescription policies.

Audit and Education responsibilities involve conducting clinical audits, answering medicine-related questions from healthcare staff, implementing medication monitoring systems, and leading educational sessions to promote quality improvements in medicine use.

In Medicines Management, the pharmacist will ensure safe prescribing practices, oversee medication storage and disposal, manage formularies, and implement NICE guidelines through audits and education.

Requirements for this role include GPhC registration, experience in primary care, expertise in medicines optimisation, strong communication and teamwork skills, and proficiency in conducting audits and improving patient care processes.
