Sheringham Medical Practice is seeking a highly skilled and autonomous Advanced Clinical Practitioner (ACP) to join our innovative and patient-focused team. Situated on the picturesque North Norfolk coast, surrounded by stunning countryside, we are a thriving dispensing practice serving 9,400 patients. Our diverse clinical team includes four partners, alongside a first contact physiotherapist, mental health liaison worker, social prescriber, Nurse Practitioners, and recovery worker. We also have a strong nursing and healthcare team with special interests in respiratory, diabetes, and women's health.
As an ACP, you will play a key role in providing expert clinical care, assessing, and prioritising patient needs, and managing caseloads, including treatment plans, referrals, and discharges. You will work closely with our multidisciplinary team to ensure the highest standards of patient care.

Key Responsibilities:
- Manage patient care from initial assessment and diagnosis to treatment and follow-up.
- Provide care for nursing and residential homes, with home visits as required.
- Deliver safe clinical decision-making and expert care in collaboration with our team.
- Experience in managing long-term conditions is advantageous but not essential.
- Candidates with specialist interests in Primary Care are encouraged to apply.
This role is suitable for Paramedics, Advanced Nurse Practitioners, or Newly qualified Nurse Practitioners.
