Are you a compassionate and skilled nurse with expertise in diabetes care? We have an exciting opportunity for a General Practice Nurse to join our team! Here are the details:

Position: General Practice Nurse (Diabetes Specialist)

Location: Silverdale Practice, Burgess Hill, West Sussex (part of Burgess Hill Villages PCN)

Hours: 16 hours per week

Pay rate: £40,000 to £41,451 per annum for WTE

Role Overview: As our Diabetes Specialist Nurse, you’ll play a crucial role in providing high-quality care to patients with diabetes. As well as providing high quality General Practice Nurse service, your Diabetes expertise will contribute to improving patient outcomes and enhancing their overall wellbeing. You’ll work closely with our Diabetic GP Lead and multidisciplinary team to deliver personalized care plans, educate patients, and support them in managing their condition effectively.

Key Responsibilities:

  • Conduct diabetes assessments and initiative insulin and provide lifestyle advice.
  • Collaborate with GPs, practice nurses, and other healthcare professionals to develop individualized care plans.
  • Educate patients on self-management techniques, medication administration, and lifestyle modifications, using appropriate technology.
  • Monitor patients’ progress and adjust treatment plans as needed.
  • Provide emotional support and empower patients to take control of their health.

Requirements:

  • Diabetes Diploma: You must hold a recognized diploma or certification in diabetes care, to include insulin initiation and GLP 1.
  • NMC Registration: Valid registration with the Nursing and Midwifery Council (NMC).
  • Passion for Diabetes Care: A genuine interest in diabetes management and patient education.
  • Excellent Communication Skills: Ability to communicate effectively with patients, families, and colleagues in a supportive and timely fashion.
  • Empathy and Compassion: Demonstrated commitment to patient-centred care.

We are a two site Practice and you will be expected to be able to work at either site and travel between the two sites.
