General Practice Nurse
Salary: £18.00 - £24 per hour - dependent on skills and experience
Reports to: Nurse Manager/Lead Nurse
We are seeking a dynamic, resilient, Practice Nurse to join our growing multidisciplinary team, consisting of GPs, ANP, Nursing Associate, Clinical Pharmacist, Specialist Chronic Disease Nurses, Paramedic, Mental Health Practitioner and HCA, together with a strong and experienced management and admin team able to support the delivery of excellent care to our patients.

Job Summary:
The post holder will be flexible, hardworking and empathetic and responsible for the delivery of practice nursing activities on the surgery premises or on specific occasions in patients own homes. They will work collaboratively with the Nurse Manager and general practice team to meet the needs of patients, supporting the delivery of quality QOF and enhanced services, Cytology, Phlebotomy, wound care, maintaining safe infection control processes, supporting the delivery of immunisation (both child and adult) and cervical screening programmes, safeguarding, policy and procedures, and providing nurse leadership duties as required.
You will be responsible for ensuring the delivery of safe and effective nursing care to the whole practice population, working closely with the Nurse Manager and Partners to deliver organisational priorities in our Medway Practices.
Clinically, the focus of the role is management of patients with long-term conditions and general nursing interventions for all patients. You will be supported by a strong MDT & clinical team working.
Perform any investigatory procedures requested by clinicians.
Participate in the administrative and professional responsibilities of the practice team, ensure accurate notes of all consultations and treatments are recorded in the patient notes.
Applicants must possess a RGN qualification, have at least one years of experience working in Primary Care, excellent ICT skills and strong interpersonal and communication skills for dealing with our diverse patient population. The post holder must also have excellent communication skills with the ability to share difficult messages to patients and families
