Practice Nurse with Leadership Skills - Low Moor Medical Practice

Position: Experienced General Practice Nurse
Location: Low Moor Medical Practice, 29 The Plantations, Bradford, BD12 0TH
Working Hours: Full-time or part-time considered

About Us:
Low Moor Medical Practice is a modern, purpose-built practice with a team of dedicated healthcare professionals providing high-quality care to 10200 patients. We are actively involved with our local LMC and are proud to be a high-achieving practice with consistently excellent QOF results. Our facilities include enhanced services like Level 2 Diabetes and attached services such as physiotherapy and ultrasound.

We have a strong, supportive team, including four GP partners, three salaried GP, three Advanced Nurse Practitioners, and a Nurse Prescriber, as well as daily in-house pharmacist support. Our practice also provides training for medical students, creating a dynamic learning environment.

The Role:
We are looking for an enthusiastic, motivated Practice Nurse with leadership skills to join our friendly, supportive team. The ideal candidate will have experience in chronic disease management and a strong commitment to patient care. If you are a proactive nurse who thrives in a busy primary care setting and enjoys leading and supporting a nursing team, this role is for you.

Key Responsibilities:

Chronic Disease Management (Diabetes, CHD, Asthma/COPD)

Women's Health, including cytology

Vaccinations and childhood immunisations

Leadership and mentorship of the nursing team

Contribution to service development within the practice

Working closely with GPs and other healthcare professionals

What We're Looking For:

Extensive experience in general practice nursing

Proficiency in managing chronic diseases

Women's health expertise, including cytology

Experience in vaccinations and childhood immunisations

Strong leadership and team-working abilities
