An exciting opportunity has arisen for an enthusiastic Health and Wellbeing Coach Soical Prescriber to join LMN PCN.
This Role involves coaching and motivating patients in managing their own health and wellbeing to live independently and improve their health outcomes. The post holder will work with individual and/or groups of patients to deliver sessions on healthy eating, physical activity and health and wellbeing to meet personalised care plans.
You will working in partnership with your clinical and non-clinical colleagues across the PCN to ensure the role delivers the best possible outcomes for patients,
