Job summary
Llandaff North Medical Centre is looking for an experience practice nurse to join our friendly clinical team, to provide high quality patient care and contribute to the development of services within our well established and thriving practice.
The post has become available due to retirement for 21 hours, working across 3 days: Monday, Tuesday and Friday. The successful applicant should be flexible.
The role would suit an experience practice nurse looking for a new challenge.
Interviews will take place on 25th September 2024.
Main duties of the job
The role involves actively managing patients on chronic disease registers. This will include asthma, hypertension, COPD and diabetes. You will be working alongside our nursing team of one practice nurse and one healthcare assistant with support from the wider clinical team.
You will provide a wide variety of clinics which will include:
Cervical Screening
Childhood Immunisations
Travel Health
Minor Illness
Wound care and dressings
HRT/Contraception reviews/counselling
Flu campaign and vaccination clinics.
About us
Llandaff North Medical Centre is a successful training practice located in Cardiff and provides healthcare to the surrounding population of around 6,500 people. The practice was built in 1997 and currently has 9 Doctors (4 partners, 3 salaried and 2 trainee GP's), 2 practice nurses, 1 Healthcare Assistant and a range of administrative staff.
The practice also has a range of NHS staff in the form of Health Visitors, Counsellors, Midwives, Pharmacist and community nurses who use our rooms to provide additional services to our patients.
We are a friendly practice with good staff retention and a strong team working culture. We offer excellent opportunities for learning and the development of new clinical skills. The clinical team meet regularly to discuss cases and ongoing CPD is actively encouraged.
Job description
Job responsibilities
To initiate and maintain disease management and health promotion. To provide general and specific health screening to the practice population (within agreed practice protocols) with referral to general practitioners as necessary. This will be within the professional competence of the post holder. Extra training will be provided before new duties are undertaken.
Working within the nursing and other clinical teams, provide a variety of clinics, always working in accordance with the NMC code of conduct.
Responsibilities include:
Responsibility for supervising health care assistants
Completing and updating templates for record information for Quality Improvement projects
Health promotion and lifestyle advice
Ensure all practice is evidence based and protocol led
Liaise with administrative staff regarding organisation of clinics
Flexibility to work as part of a multi-disciplinary team
