Acute Care Nurse Practitioner

Hours: Full time or Part time, to be discussed at interview.

Competitive salary: Ranges from £16.50 to £19.50 per hour, dependant on experience and level of qualifications

Essential: NMC registered

A&E or urgent care experience required.

The Swan Medical Group consists of five GP surgeries. The Swan Surgery in Petersfield, Liphook Village Surgery, Liphook & Liss Surgeries & Riverside Surgery in Hampshire. We aspire to be a local leader in delivering exceptional integrated healthcare.

We have a new and exciting opportunity for an experienced clinician to join our established team of Nurse Practitioners and Paramedics. The post holder will work across all sites. The team provide acute minor illness care to patients and Home Visiting Service, aiming to prevent A&E visits and hospital admissions.

We are supportive and forward-thinking practice with an excellent clinical team. We are also a training practice for doctors and are supportive of personal development & ongoing training for all staff. We aim to provide our own succession planning.

Our Practitioners are supported to work independently, autonomously delivering care and treatment to patients within the clinical setting, having gained a wide range of knowledge and clinical skills in Primary Care. These skills will enable the post holder to provide the highest standard of care.

The successful candidate will understake face to face clinical assessment and treatment of patients for minor illness, including undertaking telephone consultations and triage.

  • History taking & physical assessment degree module
  • Decision making and diagnostic module
  • Non-medical Independent Prescriber degree module
