Clinical Pharmacist for Swan Medical Group

Hours: Part time – 16 hours per week.

Competitive salary: £42,000.00 to £47,000.00 per year dependent on level of qualification and experience

Required:

  • Must be registered with the General Pharmaceutical Council
  • Must have MPharm

  • Membership of the Royal Pharmaceutical Society
  • Hold or be working towards an Independent Prescribing qualification
  • Clinical Diploma

We have a rare opportunity for a Clinical Pharmacist aspiring to work at this outstanding level. The post holder will support all 5 practices, circa 38,000 patients, by providing expertise in clinical medicines management, working as part of Swan Medical Group’s Medicines Management Team.

The Clinical Pharmacist will provide face to face or remote structured medication reviews, review of patients with polypharmacy including those within residential care homes and those with multiple comorbidities. Additionally, the role will require them to manage long term conditions, medicines on transfer of care and systems for safer prescribing, repeat prescription authorisation, and reauthorisation, as well as acute prescription requests. This is in line with the public health and social care needs of patients in the GP practices.

The post holder will also provide leadership on quality improvement and clinical audit as well as managing some aspects of the Quality and Outcomes Framework.

The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver an excellent service within General Practice.

The Post holder will be supported to develop their role and become a non-medical prescriber if that qualification is not already held. Experience and a qualification as an Independent Prescriber may secure a salary at the higher end of our salary range.
