We are looking for an Advanced Clinical Practitioner (ACP) to join our team. The post holders main responsibility will be to work within the clinical team to provide high quality patient care. The Advanced Clinical Practitioner (ACP) will use specialist knowledge and skills to provide healthcare independently to patients. This will include telephone triage, face-to-face consultations, care home visits and home visits.

Job summary:

The Advanced Clinical Practitioner will be required to work autonomously in delivering independent assessment, clinical examination, diagnosis, clinical decision making and planning of care and treatment & prescribing.

Clinical responsibilities:

  • Be responsible for the assessment of health and clinical needs of patients, undertaking consultations as an autonomous practitioner, using their advanced clinical practice skills, and own clinical judgement to diagnose, treat, prescribe and refer.
  • Be proactive in clinical decision making, underpinned by an advanced level of theoretical and practical knowledge.
  • Be included in the Duty GP rota as required by practice needs and the clinical role.
  • Maintain a high standard of patient focused care in accordance with practice’s guidelines and policies.
  • Enable the clinical team to work effectively and provide a high quality service to patients and to work effectively with other agencies.
  • Use professional judgement in managing complex and unpredictable care events and capture any learning from these experiences to improve patient care and delivery.
  • Already working as in Band 7 ACP or having achieved the relevant qualifications and with a nursing or paramedic background ,must be a Level 7 Independent Prescriber.
  • The salary for this position is Band 7.

Qualifications :

  • Atleast 1 year of nursing
  • Valid NMC or HCPC
  • Valid Imer certification
  • 2 years of paramedic
  • PG Certification
