We are looking to recruit a Chronic Disease Management Nurse to join our Practice. We are a small practice located in Keir Hardie Health Park, Merthyr Tydfil.
Our team consists of 2 GPs, a Practice Nurse, a HCSW and 6 admin staff. We have a GPSO (GP Support Officer), a Physiotherapist and a Mental Health Practitioner attached to the Practice. GP trainees complete our team as a training practice.
We are looking forward to finding a suitable candidate to work with us on 1 day a week.
