Overview:
The Rutland Health Primary Care Network (PCN) is seeking a dedicated Clinical Pharmacist to join our multi-disciplinary team. You will play a key role in supporting the PCN to deliver the Network Contract DES specifications, working primarily within Uppingham Surgery. This position offers the opportunity to complement and support our existing pharmacist team, introduce new ideas, and develop clinical special interests.
Main Duties:
- Provide proactive leadership on medicines and prescribing systems to the multidisciplinary team, patients, and their carers.
- Deliver patientfacing services and clinics that improve patient outcomes and reflect the needs of the practice population.
- Enhance patient and carer understanding, confidence, and compliance with their medicines.
- Conduct pharmaceutical consultations and home visits for patients with longterm conditions.
- Perform highquality medication reviews and develop bespoke medicines treatment plans for elderly, complex patients, and those at high risk of admission.
- Improve prescribing safety and quality following hospital admissions.
- Conduct clinical audits and education to improve performance against NICE standards and other clinical guidelines.
- Develop the role through clinical supervision, training, and service redesign activities.
- Maintain active links with community pharmacists and other stakeholders to improve patient experience and manage issues.
- Provide training, mentoring, and guidance to other clinicians and staff on medicinerelated issues.
Structured Medications Review and Optimisation:
- Tackle overmedication, inappropriate antibiotic use, and withdraw unnecessary medicines through NHS-led programs like low priority prescribing and medicines optimisation.
- Work enhanced access hours (6:30 pm 8:00 pm) one day per week.
- Access CPPE and IP training if not already qualified.
