Part-time experienced Primary Care Pharmacist
MPA GP PRACTICES – based at Castle Medical Centre, but may be at other sites in Medway if needed
Salary - depending on experience and prescribing qualification
NHS Pension available
Hours – Part-time (24 hours over the week)
Job summary:
We are looking for an enthusiastic, experienced Primary care pharmacist to join our team, and to work on-site at our GP practice at Castle Medical Centre, Rochester which has about 5000 patients. MPA is its own PCN with 39,000 patients over 3 contracts and 7 sites.
You will be well supported as our clinical team currently has five prescribing pharmacists, two junior pharmacists and a Medicines Management administrator plus GPs, ANPs, Physician associate, paramedics, physiotherapist, podiatrist and practice nurses. We also host a foundation year pharmacist for six months of each year with Medway Foundation Trust.
We are looking for an experienced primary care pharmacist preferably with a prescribing qualification. If you are not yet a prescriber we can support the right candidate to complete the course. We are keen to support a pharmacist who has a specialist area of interest to help provide the best care to our patients, or we can support you to expand into an area of your choice.
Your main duties will involve: face-to-face clinics to undertake medication reviews and chronic disease management; drug audits for clinical safety and quality improvement and to help meet local ICB targets; medicines reconciliation after hospital discharge; dealing with medication-related queries from patients and clinicians; helping with repeat medication requests; and other clinical work depending on your experience.
