An exciting opportunity has arisen at Swanley Medical Practice for the recruitment of a highly motivated and enthusiastic Practice Nurse to join our welcoming and well-supported team.

Situated in the town of Swanley, conveniently located just off the M25/A20, Swanley Medical Practice is an integral part of the NHS, dedicated to providing high-quality healthcare services to our community. Our practice operates across three sites: The Cedars and The Oaks in Swanley, approximately two hundred meters apart, and the third in the village of Hextable. With a patient list of 24,000, we boast a dedicated team, including GP Partners, Salaried GPs, Clinical Pharmacists, Advanced Nurse Practitioners, Nurses, HCAs, and other clinicians from our Primary Care Network. Notably, we are also a GP Training Practice. We are looking to build on our existing Nursing team, we are currently training a Nurse Associate and have three experienced nurses who have joined the practice in the last few years.

Job Title: Practice Nurse

Location: Swanley Medical Practice, Nightingale Way, Swanley BR8 7UP

Type: Full-time or Part-time (Flexible hours)

Job Overview:

As a Practice Nurse at Swanley Medical Practice, you will be responsible for a number of clinical areas such as health promotion, chronic disease management and illness prevention. You will contribute to the smooth running of appointments, improving patients experience in the surgery.

Key Responsibilities:

Patient Care: Responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team.

Administrative Support: Manage appointments, accurate maintenance of clinical records, effective use of SNOMED codes, and the management of chronic disease registers. Maintaining records such as baby immunizations and vaccinations. Efficiently manage phone inquiries and provide excellent patient service.

Clinical Care: The responsibilities include identifying, managing, and supporting patients at risk of developing long-term conditions to prevent adverse effects on their health. Routine nursing care according to clinical-based evidence, NICE, and the NSF guidelines. Providing vaccinations/immunisations and travel medicine, wound care, requesting necessary pathology services with subsequent result processing. Conducting chronic disease clinics, delivering patient care and making referrals to secondary/specialist care when required. Prioritizing health issues, recognising, and referring patients with mental health needs, and supporting patients in medication use. Safeguarding matters, delivering opportunistic health promotion and the management of emergency drug kits.

Health Promotion: Support health promotion initiatives within the practice by providing information to patients on healthy lifestyle choices and encouraging patients to engage in preventive healthcare measures. Contribute to patient education programs within the practice.

Infection Control: Adhere to infection control protocols to maintain a safe and clean environment for patients and staff. Assist in maintaining the cleanliness and organization of clinical areas.

Team Collaboration: Collaborate with healthcare professionals, including GPs, nurses, and administrative staff, to ensure smooth operations within the practice.

Requirements:

Registered Nurse qualification and registration with The Nursing and Midwifery Council with a valid NMC PIN

Excellent communication and people skills.

Ability to work effectively in a demanding environment.

Ability to work both independently and as part of a team.

Strong organizational skills and attention to detail.

Willingness to learn and adapt in a fast-paced healthcare environment.

Compassionate and patient-centred approach to care.
