Bournemouth East Collaborative Primary Care Network (PCN), situated on the stunning Dorset South Coast, comprises four like-minded practices working together in East Bournemouth, with a strong reputation on quality improvement and investing in its employees. The PCN serves a population of approximately 55,000 patients with a diverse demographic.

Job summary

Our Enhanced Care Team is a multi-disciplinary team based at Southbourne Surgery that provided support to our aligned Care Homes, our proactive frailty support and acute visiting service to housebound patients.

We would ideally like an Advanced Clinical Practitioner to work full time alongside the Lead GP and Lead ANP for the Enhanced Care Team, deputising for the Lead ANP when required. The role will predominantly deliver acute visits to our housebound patients but will also be involved in the training and support of our aligned Care Home staff, as well a broader education and development for the communities served by the PCN.

Advanced Practice is a level of practice rather than a type of practice and the Advanced Nurse Practitioners within Bournemouth East Collaborative PCN have the freedom and authority to act, making autonomous decisions in the assessment, diagnosis, and treatment of patients.

Advanced Clinical Practitioners working at this level of education and training have significant roles within each of the following four pillars, and will be expected to help develop the role within the organisation: -

• Advanced clinical practice

• Leadership

• Facilitation of education and learning

• Evidence research and development
