An exciting opportunity has arisen for Three Care Coordinators to join our practice as part of our multi-disciplinary care team. We pride ourselves on providing excellent patient centred care and wish to expand our team to deliver high quality personalised care to patients.
The successful candidate will work within the multi-disciplinary care team and will focus on supporting the development of proactive personalised care planning including advanced care planning. The Care Coordinator role involves working with the GP's and the multidisciplinary teams (MDT) and will support them to coordinate all key activity including improving access to services, providing advice, and information, and ensuring health and care planning is timely, efficient, and patient-centred.
The candidate must hold a certificate in care coordinator and have previous primary care experience.

Our practice has a patient list size of over 12,500 and the lead practice for the PCN in a group of Three surgeries, we work to deliver improved services to our population.
We have a large multi-skilled team consisting of 2 GP Partners, 6 Salaried GPs, 1 Clinical Pharmacist, 2 Practice Nurses, 1 Trainee Nursing Associate, 2 healthcare assistants, and a phlebotomist who are supported by a large administrative team including an assistant practice manager, and reception supervisor.
The position is full- time.
