It is an exciting time to join primary care; The NHS Five Year Forward View (2014) put GP surgeries at the heart of the vision for improved healthcare. The transformation of local healthcare and the formation of Primary Care Networks (PCNs) has provided the opportunity for pharmacy teams to work alongside a range of health professionals to play a key role enhancing health, improving safety, and advancing quality.

Wyvern Health Partnership (WHP) is a PCN of 5 surgeries in Swindon, supporting approximately 52000 patients. The partnership incorporates Hawthorn Medical Centre, The Lawn Medical Centre, Merchiston Surgery, Old Town Surgery and Priory Road Medical Centre.

You will join the PCN’s clinical team of GPs, Nurse Practitioners, Nurses, Health Care Assistants, Nursing Associate, Pharmacists, Pharmacy Technicians, First Contact Physiotherapists, Social Prescribers, Mental Health Nurse and Dietician.

The Specialist Practitioner will utilise skills such as those of paramedics to support the first contact needs of patients in unscheduled care. Supported by GPs and triage teams from the practices, the Specialist Practitioner will triage home visits to either facilitate an early presentation in A&E, thereby reducing the likelihood of admission, or referral to other services as necessary. The Specialist Practitioner will independently provide definitive care that may not require further intervention of a GP.
