Please note this is an HCA type of role (not a care worker role) so please do not apply if you have no phlebotomy experience as a minimum

This is an exciting opportunity to join a vibrant, fast moving Primary Care Network, covering the Pudsey / Bramley area. The role is a blend of community (domiciliary) work and clinics on site across our 7 practices. No two days are the same and if you have a desire to be upskilled we are very happy to accommodate that. This role goes beyond that of a General Practice HCA; clinical skills will prove to be just as important as relationship building skills (both with patients and staff across our 7 practices). We already have a well developed and established team of HCAs (Primary Care Support Workers) so you will be joining an organisation with great peer support. The workload is also very diverse so if you like variety this is probably the role for you

Primary Care Networks were introduced in the NHS Long Term Plan published in 2019 to address Health Inequalities, Population Health Management and to provide Enhanced Clinical Services on an economies of scale basis. West Leeds PCN already has a significant workforce covering many different clinical disciplines which means that in-house MDT working is part of our work ethic; this benefits our patient population enormously.

We will consider part time hours for the right candidate (minimum 30 hours). The salary advertised is the Whole Time Equivalent.

Main duties of the job

You will work as part of a multidisciplinary team making home visits across a population of patients who are isolated, frail, vulnerable or housebound; you will carry out routine screening. You will also have days based in clinic within our surgeries. You will manage and prioritise your own caseload under supervision and in accordance with the care needs, priorities and any urgent support required by individual / patient.

You may also be needed to assist with the ongoing Covid vaccination programme. There will also be other ad hoc tasks you may be asked to fulfil.
