JOB SUMMARY
Working as part of the PCN multi-disciplinary team at an advanced level in the management of patients living with frailty, long term conditions or complex needs. To lead a PCN Team to provide an effective, efficient service for the chosen population.
The multi-disciplinary team comprises of Nurse Associates, HCA’s, Care Coordinators, Social Prescribers the role will require Leadership and Managerial responsibilities to enable the PCN’s provide an effective team for the chosen population.
Using expert occupational therapy knowledge and advanced clinical reasoning skills you will provide specialist support and advice to a team of care coordinators and other associated healthcare professionals and partners. This post will help provide a quality, effective management programme providing personalised care plans, to help prevent avoidable hospital admissions and help safe discharge from hospital.
In addition to working within the PCN teams, you will also work with the PCN GP Practices and other health and social care organisations.
As part of this role you will be expected to work across various sites and settings within the community to fulfil the requirements of the role.
You will be working towards the delivery of the outcomes outlined in the Primary Care Network Contract Enhanced Service Specification.
The post holder will be employed by South East Leeds GP Group, but will be deployed and managed day to day from within the Primary Care Network
