GPwSI in Older Persons Care
We are looking for a GP or Specialist Grade doctor with a special interest in community geriatric medicine.
We are a friendly, happy, large and well supported department, and we’ve just been given the exciting opportunity to further develop our community service. We have the highest GMC score for training in the South West, close links to both Bristol University and Peninsula Medical Schools, and a well-established mentoring scheme.
We’re open to discussing job share or less than full time arrangements. This post is genuinely flexible. You will be working with an established community teams including Rapid Response and Discharge to Assess, alongside a new team of Advanced Clinical Practitioners, with the support of geriatricians based in our Acute Frailty Unit.
The care of frail older people is a ‘flagship’ project for the NHS in Somerset.
This is an exciting opportunity to help shape care for older adults across our county. Frailty is a team effort: everyone here is committed to the successful delivery of services across the whole of Somerset with proactive collaborative working between primary and secondary care in community and acute settings. Our work is supported by the National Frailty Network, and is a priority programme across Somerset, alongside other initiatives including Anticipatory Care.
Our clinical strategy will increase the time people have to do the things that matter to them, focussing on more time in good health whilst eliminating wasted time in health care. Somerset Foundation Trust is the first Trust in England formed of acute, mental health and community services and offers clinicians an incredible opportunity to shape pathways of care.
In addition to joining a Trust with ratings of Good and Outstanding from CQC, and the highest GMC score in the South West for training, you will get to experience life in the West Country, taking advantage of easy access to some of the UK’s most desirable locations. Nestled in the heart of Somerset as the County town, Taunton has a diverse range of shopping and restaurants, and boasts sporting arenas such as the county cricket ground regularly hosting international fixtures, as well as national hotspots for cycling, surfing, and climbing such as Cheddar Gorge. Straddled between areas of outstanding natural beauty and the idyllic rolling hills of Exmoor, there’s plenty to keep you occupied alongside the abundance of music festivals and local traditions.
The county town is Taunton has an estimated population of 88,000 and is primarily an administrative centre and sporting centre. The other main towns are Bridgwater in the North West and Yeovil in the East of the county. Bristol is within easy reach by road and there is a fast train service to London from Castle Cary and Taunton.
