Do you want a role with a mixture of remote and practice working. We are looking for General Practitioners to work for Sussex Primary Care in a hybrid role which will provide extended hours support to the practices in Sussex Primary care and wider PCN's. This will include some weekend work at an enhanced rate. You will have the flexibility to take a hybrid approach, work from home and in Dolphins Practice located in Haywards Heath. A flexible working pattern throughout the week, to include rotational Saturday mornings. Main duties of the job To be responsible for providing primary medical services to the registered patients of the practice, ensuring the highest standards of care for all. This will be undertaken in line with the relevant national primary care contractual regulations and guidelines. Manage emergency/immediate necessary and temporary patients. Have the opportunity to take part in leading on an area of interest. Support the CQC Registered /Practice Manager in delivering quality, risk and regulatory compliance agenda to meet local, regional and national requirements. Provide health promotion and advice, signposting patients to appropriate services to maximise their health and social functioning and their ability to self-manage their care. Be constantly mindful of the need for the provision of both personalised care and population health, including reducing health inequalities and ensure that these are reflected in daily practice. Job responsibilities In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care as set by the regulatory and standard setting organisations, including but not limited to the points below. In accordance with the practice timetable, as agreed, the post holder will undertake a variety of duties including: a) Providing to a registered list of patients: surgery consultations and emergencies, telephone consultations and queries, triaging, visiting patients at home, prescribing (including checking and authorising repeat prescriptions and dealing with queries), patient test results, patient medical reports and examinations on behalf of the Practice as well as referral letters and other paperwork and correspondence in a timely manner. b) Provide health promotion and advice, signposting patients to appropriate services to maximise their health and social functioning and their ability to self-manage their care. c) Be constantly mindful of the need for the provision of both personalised care and population health, including reducing health inequalities and ensure that these are reflected in daily practice. d) Make professional, autonomous decisions in relation to presenting patients, whether self-referred or referred from other health care workers within the organisation or external organisations. e) Assess the health needs of patients with undifferentiated and undiagnosed problems taking appropriate action as necessary. f) Screen patients for disease risk factors and early signs of illness taking appropriate action as necessary. g) In consultation with patients and in line with current disease management protocols and care pathways, develop care plans for health related illnesses and share these across the multi-disciplinary team, in line with patient need and good information governance. h) Record clear and contemporaneous consultation notes to agreed standards using IT systems. i) Compile and issue computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible) including EPS. j) Prescribe in accordance with locally agreed or nationally agreed prescribing formulary. k) Work in accordance with all relevant clinical guidelines, pathways and protocols, including agreed standard data entry processes. l) The post holder may have tasks or responsibilities delegated to them, appropriate to their level of competence. They may also be expected to delegate tasks or responsibilities to other staff, as appropriate.
