Hours: Up to 37.5 hours
Salary: Upwards of £53,000 per annum, depending on knowledge, skills and experience
To support our continued growth, we are looking for a Senior Advanced Clinical Practitioner to join our multi-disciplinary team at Highbridge Medical Centre, which is part of Symphony Healthcare Services (SHS), the largest at-scale provider of primary care services in Somerset, supporting 20 practices across Somerset (including one in Devon) and providing care for 120,000 patients. As an Advanced Clinical Practitioner, you’ll be expected to assess all patients and provide the right management or referral. You will be reliable and dependable, passionate and committed to delivering high quality health care in a primary care environment. You will be willing to contribute to the processing of administration tasks including and not limited to signing repeat prescriptions, laboratory results and clinical letters from other providers.
Our practice has an established MDT management team and we can support you as an ACP to lead on key areas of service according to your specialist areas of interest. As a practice we are developing a strong ACP workforce who are encouraged to be involved in digital transformation, clinical research and are supported to lead quality improvement initiatives.

Highbridge Medical Centre is a large, well established practice with an on-site pharmacy. Our integrated, multidisciplinary team includes GPs, nurse practitioners and ECPs and provides a wide range of services to around 12,000 patients.

Main duties of the job
Job responsibilities
You will work as an advanced, autonomous practitioner using a high level of decision making. Your primary focus will be the provision of excellent patient centred care, encompassing the skills of face to face clinical assessment, examination, clinical diagnosis and the development and delivery of effective urgent treatment for patients of all ages with undifferentiated, undiagnosed primary/ urgent health care problems of full general practice patient needs including paediatrics, care of elderly and mental health.
This role works within the parameters of current prescribing legislation.
You will be part of a multidisciplinary and required to deliver face to face consultations from the practice or within patients own homes when required.
This role is particularly suited to an autonomous Clinician that has undertaken core modules within the Specialist Paramedic, Nurse/Paramedic Practitioner program or an experienced Nurse/Paramedic who has additional training in assessing minor illness and is a non-medical prescriber.
You will receive a full induction and be supported by our Clinical Lead.
The successful candidate will:
- be an experienced practitioner acting within their professional boundary to provide care for the presenting patient. This role is expected to develop and evolve over time as the successful post holder gains further experience within the primary care setting.
- be flexible and undertake any necessary additional training required to meet the service needs.
- work collaboratively with the multidisciplinary team to advise on treatment and healthcare for patients, to include those presenting in the primary care setting and those in the community.
- attend home visits to include residential and nursing homes, where they will liaise with staff, family and other stakeholders in the production of care plans to be shared with other providers, as appropriate.
- assess and treat acute and chronically unwell patients presenting in the surgery environment.
