Post: PCN Care Coordinator
East Suffolk Primary Care Network is an innovative partnership of 11 practices providing primary healthcare to more than 150,000 patients. The collaboration sees doctors and other practice staff sharing resources and working together to ensure patients continue to receive the highest quality healthcare.
We are seeking to recruit into five posts, which will be split across GP Practices in Felixstowe, Martlesham and Felixstowe.
This is a new role that has a focus on administratively supporting practices across the PCN as well as helping to proactively identify and communicate with patients who may benefit from our help and support.
The PCN is developing a wide range of services and you will play a particularly important role in helping to setup and develop projects.
You must have strong I.T. skills, particularly in Microsoft Office. Experience of working with GP Clinical systems would be ideal but not essential You should be motivated and be able to evidence excellent written and verbal communication skills.
Experience in health care would be an advantage but is not essential if you can show an understanding Primary Care.
