The Mayflower Group is looking to recruit an enthusiastic Nurse/Paramedic who has experience working with patients in a community setting and as part of a multidisciplinary healthcare team. This is a new and exciting opportunity to become part of our nursing team in a primarily patient-facing role, with the chance to utilise your clinical skills to improve the health of the local community supporting patients residing in nursing homes and those who are housebound.

The role is aimed at improving continuity of clinical care, reducing unplanned hospital admissions & reduced GP Visits. You will undertake proactive care planning for care home patients, resulting in risk reduction and provide care in a local setting by having regular contact with nursing homes, managing long terms conditions and complex cases.

This post represents a development opportunity for you to develop your clinical skills, and you’re your prescribing qualification (if needed) whilst shaping a new and exciting service. You will be part of a close-knit team, and will function with a high degree of leadership, authority and autonomy focussing on patient-centred care. This post will be supported by supervision, on-going personal and professional development and training as appropriate to meet knowledge and skills gaps.

The responsibilities will include, but are not limited to the provision of planned on going care for our patient group:

• Management of housebound and Nursing Home patients across the Mayflower Group

• Assisting in the reduction of GP visits and hospital admissions

• Maximising QOF potential across this particular patient group

• Working closely with the Acute Care Team and other clinicians to manage demand

We are looking for candidates who are adaptable to change, with excellent clinical skills and a good level of competence in the use of IT. Candidates should be able to demonstrate excellent organisational skills and an ability to communicate effectively with a wide range of health professionals.

This position is for 40 hours per week and the rate of pay is £22.45 per hour.
